This document serves as a memo of understanding between WorldAPP, Inc. and (company name) in regards to the 60 Day Enterprise Evaluation being provided by WorldAPP, Inc. to (company name).

The following is considered to be understood:

The purpose of the trial is to allow (company name) to fully test all features and functionality of the Key Survey application in order to assess if it serves their specific survey application requirements.

All functions and features of the Enterprise Version will be made available to (company name) during this period, including the right to launch active surveys through the application’s email distribution function.

(Company name) will incur no cost or financial outlay for this 60 day evaluation.

Signing this document does not constitute any binding or legal commitment whatsoever on the part of (company name) to participate in any commercial transaction with WorldAPP, Inc. upon termination of this 60 day Enterprise evaluation trial period.
